The Minnesota Department of Human Services (DHS) has made changes to plan options for Ramsey County residents applying for, or enrolled in, Special Needs Basic Care (SNBC).
Health Partners will no longer be an option as of April 1, 2025.
If you currently have Health Partners insurance, you will need to choose a new plan: Medica or UCare.
If a new plan is not selected, you will be randomly enrolled into one of these plans.
