2025 Managed Health Care Updates
The Minnesota Department of Human Services (DHS) has made changes to the Managed Health Care (MHC) plan options for Ramsey County residents applying for, or enrolled in, Medical Assistance (MA) or Minnesota Care (MCRE) programs.
If you are enrolled in Medical Assistance or Minnesota Care programs, please review changes for Health Partners, UCare and UnitedHealthCare health plans.
For Minnesota Care applicants or enrollees, please call DHS at 651-431-2670 for plan options in Ramsey County.
For new enrollees in 2025

Available plans for new enrollees include: Blue Plus and Medica.
For residents enrolled in Health Partners or UCare
- People enrolled in Health Partners and UCARE who are not enrolled in SNBC can stay with their same coverage.
- You still have the option of changing plans during the Annual Health Plan Selection (AHPS) period, to Blue Plus or Medica for 2025.
The AHPS deadline was Dec. 19, 2024, but it will be extended. The new deadline has not been decided by DHS yet, but you are strongly encouraged to make your selection as soon as possible.
For residents enrolled in UnitedHealthcare
- UnitedHealthcare is no longer an option effective Dec. 31, 2024.
- You must select a new plan, or one will be randomly assigned to you and your household members.

For residents enrolled in Special Needs Basic Care (SNBC)
- Health Partners will no longer be an option as of April 1, 2025.
- If you currently have Health Partners insurance, you will need to choose a new plan: Medica or UCare.
- If a new plan is not selected, you will be randomly enrolled into one of these plans.
- All questions for SNBC should be directed to the Disability Hub at 866-333-2466.

I’m enrolled in Health Partners, UCare or United Health Care. I’m pregnant and due in January. What are health plan options for my newborn?
Newborns will be added to the same health plan as their mother.
I want to change health plans, but I’m not sure if my current clinic will accept a new plan.

I’m enrolled in Health Partners and don’t want to change. What are my options?
For Prepaid Medical Assistance Program (PMAP) (non-disabled between 0-64yrs), you may remain with Health Partners throughout 2025. For SNBC: Health Partners will end April 1, 2025 so you must select a new plan. If you do not select a new plan, you will be automatically enrolled in a different plan.
I filed a new application and was previously enrolled in Health Partners or UCare in 2024. What are my options?
For PMAP, you may be re-enrolled into Health Partners or UCare for 2025 as this is an exception. Or choose one of two new options: Blue Plus or Medica.

I’m currently enrolled in UnitedHealthcare, what are my options?
You must select Blue Plus or Medica as your health plan for 2025. If you do not take action, you will be randomly assigned to one of these health plans.
