The 2024 Heading Home Ramsey Needs Assessment, conducted from June to November, fulfills requirements for federal and state funding applications. It evaluates Ramsey County’s homelessness response system through the Heading Home Ramsey Continuum of Care (CoC), which oversees more than 100 programs across 14 cities.
This assessment highlights the CoC’s structure, governance and funding coordination, as well as service pathways from prevention to housing stability. It underscores gaps in the system and examines homelessness as a public health crisis intensified by economic and pandemic-related challenges, emphasizing the critical role of policy in preventing housing instability.
System Strengths
The system’s strengths include a committed network of providers, a shared vision, and a wealth of institutional knowledge. These strengths help identify key gaps, such as insufficient prevention funding, a shortage of shelter beds, and limited diversity in housing programs.

However, the response system faces significant challenges, including a lack of affordable housing, limited turnover in permanent supportive housing, and the strain of post-pandemic funding reductions. These issues lead to extended wait times, longer shelter stays, and reduced system flow. Racial disparities in homelessness rates also demand focused investigation and culturally specific programming.
Provider Challenges
Providers face workforce shortages, high staff turnover, and insufficient transportation resources for clients. System performance measures reveal concerning trends, including rising first-time homelessness cases and increasing returns to homelessness.
Solutions
To address these challenges, the report found the CoC must prioritize equity, increase prevention and housing funding, diversify programs, and enhance collaboration among providers. Strengthening system performance and reducing disparities will create a more effective and compassionate homelessness response system.
