COVID-19 Employee Certification of Vaccination Status & Notice of Testing Requirements

Ramsey County is committed to providing a safe and healthy workplace for all employees and preventing the spread of COVID-19. To ensure a safe and healthy workplace, the potential for transmission of COVID-19 while working must be limited through the cooperative efforts of all employees.

All county employees are encouraged to be fully vaccinated against COVID-19. Employees who do not provide proof that they are fully vaccinated against COVID-19 must be tested on a regular basis for active infection as a critical tool for minimizing potential exposure to COVID-19 and preventing further transmission of the virus.

If you are fully vaccinated against COVID-19, the county is requesting you to present your COVID-19 Vaccination Record Card or other sufficient proof of vaccination. This information will be used to confirm whether you are fully vaccinated against COVID-19, and whether you will be required to complete mandatory COVID-19 testing.

You are not legally required to provide your vaccination status or proof of vaccination. However, if you do not provide the data, you will be considered unvaccinated against COVID-19, and you will be required to provide weekly COVID-19 test results as required by Ramsey County COVID-19 Proof of Vaccination and Testing Policy. If you do not show proof of full vaccination against COVID-19 and fail to participate in routine COVID-19 testing as required by the policy, you may be subject to disciplinary action, up to and including discharge.

Ramsey County will not tolerate retaliation, discrimination, and/or harassment based on vaccination status or for speaking out about unsafe working conditions or reporting a work-related illness or infection or exposure to COVID-19.

The Minnesota Government Data Practices Act classifies your vaccination status as private data about you. The data collected from you will only be shared with HR staff or their designee to administer the program as well as other staff with a business need to know, and other persons or entities authorized by law.

Within Summit, employees must complete the certification truthfully and to the best of their knowledge.

  • Yes – I am fully vaccinated.
    • By selecting “Yes", you are certifying that you are fully vaccinated and will upload proof of vaccination on the Vaccinations page in Summit Self-Service. Fully vaccinated means that you have received two-shots in the two-part series (Pfizer or Moderna) or one-shot in the one-shot series (Johnson&Johnson/Janssen) and you provided proof of vaccination.
  • No – I am not fully vaccinated.
    • By selecting “No”, you are certifying that you are unvaccinated, partially vaccinated, or you do not wish to disclose you are fully vaccinated.

Unvaccinated means that you have not received any vaccination against COVID-19. Partially vaccinated means that you have received at least one-shot of the two-shot series (Pfizer or Moderna). You may upload proof of your partial vaccination on the Vaccinations page in Summit Self- Service. Employees who select “No” must upload weekly COVID-19 test results to the Test Results page in accordance with the COVID-19 Proof of Vaccination and Testing Policy.

Employees who do not certify to their vaccination status will be considered unvaccinated and must upload weekly COVID-19 test results to the Test Results page in Summit Self-Service in accordance with the COVID-19 Proof of Vaccination and Testing Policy.
