Vehicle Accident Reports

The Minnesota Law Enforcement State Accident Report is not the same as the Minnesota Motor Vehicle Crash Report (PDF). Every driver in a crash involving $1,000 or more in property damage, injury or death, must download, complete and mail in the Minnesota Motor Vehicle Crash Report (PDF).

This completed form must be sent to this address within 10 days:

Failure to provide this information is a misdemeanor under Minnesota Statute 169.09, subdivision 7.

When requesting a report, please provide a case number (CN) whenever possible. Case numbers help expedite the process. If a case number is not available, please provide as much information as possible (date, time, location, names of persons involved, etc.). 

In-person requests

Appear in person at the Sheriff’s Patrol Station (located at 1411 Paul Kirkwold Drive, Arden Hills, Minnesota 55112). Patrol Station hours are Monday through Friday, 8 a.m. to 4:30 p.m., excluding holidays.

The request must include a:

• A self-addressed, stamped envelope 
• A money order made or check payable to Ramsey County

Allow 4-6 days for delivery.

Fees

Credit/debit cards are not accepted. Cash, check, and money orders are accepted. Money orders must be payable to: Ramsey County.

A fee of .25 per page is charged.
